The TCR and IGH genes undergo rearrangement at the DNA level during maturation of T- and B-lymphocytes (respectively). Analysis of these genes can detect a clonal proliferation of T-cells and/or B-cells, and thereby provide diagnostic information.
T-cell or B-cell gene rearrangement PCR can used to help distinguish between polyclonal (non-neoplastic) and clonal (usually neoplastic) lymphoproliferation.
Some clonal lymphoproliferations are not neoplastic, and some neoplastic clones will not be detected by this test. The results of this test must be considered in the overall clinical context of the patient.
This is an assay for non-heritable mutations. It does not raise issues of ethics or consent that are different from most other investigations ordered in the routine care of a patient.

This test can be requested by any medical practitioner.
4-10 mL of bone marrow in dedicated EDTA, 10 mL peripheral blood in dedicated EDTA, or sections from formalin-fixed, paraffin-embedded (FFPE) tissue.
To help ensure the quality of the test, a genetic test should be done with a dedicated sample whenever possible i.e. a sample collected specifically for that test rather than a sample that is used for multiple tests.
We also recommend that the patient or another adult check the labelling of request forms and sample tubes.
